ADULTS:
It includes physical examination, test for syphilis, tuberculosis (in blood) and gonorrhea (in urine) and filling in the form.
IT DOES NOT INCLUDE: Vaccines, X-rays or any other necessary tests.
CHILDREN:
It includes physical examination, tuberculosis test (blood) and filling out the form.
IT DOES NOT INCLUDE: Vaccines, X-Rays or any other necessary tests.
REQUIREMENTS:
All patients, children or adults, must bring their passport, if you already have an alien number you must bring it ( it appears on the work permit and in letters received from USCIS)
All patients, children or adults, must bring their immunization record or card, If you do not have it or cannot obtain it, you must have the corresponding vaccines or show proof of immunity.
Please verify that your address and telephone number are up to date in our clinic registry.
VACCINES:
USCIS requires that all patients under the age of 18 have the complete immunization schedule recommended by the CDC for their age and the complete COVID-19 immunization schedule if applicable.
USCIS requires all adults to have the following vaccines:
Current season influenza (FLU)
TDAP ( applied within the last 10 years)
MMR
CHICKENPOX
And the full COVID-19 VACCINE SCHEDULE.

REQUISITOS:
Todos los pacientes, niños o adultos, deben traer su pasaporte, si ya tiene un número de extranjero debe traerlo (aparece en el permiso de trabajo y en las cartas recibidas de USCIS)
Todo paciente, niños o adultos, debe traer su cartilla o carné de vacunación, en caso de no tenerla, debe aplicarse las vacunas correspondientes o presentar prueba de inmunidad.

VACUNAS:
